States Having The Best (And Worst) Pet Healthcare In America: New study shows Colorado leads the nation in pet care quality while Delaware ranks last – how does your state measure up?

Behind these differences are factors like the number of licensed veterinarians, availability of emergency clinics, and insurance rates – all of which can make the difference between life and death in a pet emergency.

“Pet owners often don’t realize how much their location affects their ability to get quality care for their animals until it’s too late,” says Nick Jordan, Founder of Your Service Animal.

“Understanding these disparities helps families make better decisions about their pets’ health needs.”

To help pet owners understand these disparities, Your Service Animal, which helps people obtain legally compliant Emotional Support Animal letters, conducted a comprehensive study analyzing pet healthcare quality across all 50 states.

The study analyzed five key factors in each state: licensed veterinarians per 100,000 residents, availability of 24/7 emergency clinics, costs of both routine and emergency care, and percentage of pets insured. These metrics were normalized and weighted to create a composite Pet Healthcare Quality Score score ranging from 0 to 100, with states ranked accordingly.

Colorado: The Gold Standard for Pet Care

Colorado stands head and shoulders above other states, scoring a perfect 100 on the Pet Healthcare Quality Score index. The Centennial State boasts an impressive 106.98 veterinarians per 100,000 residents, more than double California’s availability.

Colorado also leads in emergency care access with 0.17 emergency clinics per 100,000 residents, the highest in the nation. The combination of excellent professional care access and moderate costs ($71.95 for routine care and $45.49 for emergency services) makes Colorado the clear leader in pet healthcare.

“Colorado’s strong performance is a reflection of its outdoor-oriented culture where pets are considered core family members,” says Jordan. “The state has invested in veterinary education and created an environment where animal healthcare professionals want to practice.”

California: Pet Insurance Capital

California secured the second spot with a score of 64.96, despite having fewer veterinarians per capita (37.94 per 100,000) than other top states. What propels California near the top is its extraordinary pet insurance adoption rate – a stunning 18.6% of pets are insured, more than six times the national average. The state also ranks second in emergency clinic availability (0.105 per 100,000 residents) despite facing higher costs for both routine ($84.59) and emergency care ($46.99).

“California pet owners understand the value of insurance in managing high care costs,” notes Jordan.

“The state’s progressive approach to pet ownership really shows in the number of families choosing to protect their pets financially.”

Arizona: Desert State, Oasis of Care

Arizona rounds out the top three with a score of 62.38, powered by the third-highest concentration of veterinarians (85.62 per 100,000 residents). The state balances good emergency care access (0.067 clinics per 100,000 residents) with some of the most affordable care costs in the top ten – averaging just $67.95 for routine care and $28.10 for emergencies.

“Arizona demonstrates that quality pet healthcare doesn’t have to break the bank,” says Jordan. “Their model of high professional availability combined with reasonable costs creates excellent access for pet owners.”

Delaware: Pet Healthcare Desert

With the lowest possible score of 0.00, Delaware faces significant challenges in pet healthcare access. The First State has zero 24/7 emergency clinics, just 38.11 veterinarians per 100,000 residents, and a low pet insurance rate of 0.4%. Combined with relatively high routine care costs ($78.95), Delaware pet owners face a challenging combination of limited access, few emergency options, and financial vulnerability.

“Delaware’s situation highlights how geographic factors can create healthcare deserts even in smaller states,” Jordan explains. “Pet owners there often need to cross state lines for emergency care, creating dangerous delays in treatment.”

Alaska: Isolation Creates Barriers

Alaska ranked second-worst with a score of just 9.29, suffering from similar challenges to Delaware. The Last Frontier has no 24/7 emergency clinics and just 29.99 veterinarians per 100,000 residents. While costs are moderate ($70.42 for routine care and $17.61 for emergencies), the extreme geographic isolation means many pet owners must travel long distances for care.

“Alaska’s vast size and remote population centers create inherent challenges for pet healthcare delivery. The lack of emergency options is particularly concerning given the state’s extreme weather conditions,” says Jordan.

West Virginia: Limited Resources

West Virginia rounds out the bottom three with a score of 15.83. Like other low-ranking states, it has zero 24/7 emergency clinics, forcing pet owners to travel across state lines for after-hours care. With 43.03 veterinarians per 100,000 residents and minimal insurance adoption (0.4%), the Mountain State struggles to providecomprehensive pet healthcare.

Regional Patterns and Urban-Rural Divide

The data reveals clear regional patterns, with Western states dominating the top rankings (Colorado, California, Arizona, Wyoming) while Northeastern and Midwestern states feature prominently in the bottom rankings. There’s also a noticeable urban-rural divide, but not in the expected direction – many rural states like Wyoming (56.97) and Vermont (52.42) outperform more urbanized states like New York (19.88) and Illinois (18.60).
“What’s interesting is that rural states often compensate for fewer emergency facilities by having more veterinarians per capita,” Jordan points out.

“Wyoming has zero 24/7 clinics but boasts 130.95 veterinarians per 100,000 residents – the highest concentration in America.”
The cost analysis shows no clear correlation between expenses and quality. New York has some of the highest costs ($91.95 for routine care) but ranks near the bottom (#45), while Nebraska offers some of the most affordable care ($60.95 for routine visits) while maintaining a strong #7 ranking.

When pets lack access to timely care, preventable conditions often become emergencies, which results in higher costs and worse outcomes. For those in low-ranking states, I advise establishing a relationship with a primary vet before emergencies occur, researching the nearest 24/7 facilities (even across state lines), building an emergency fund, and considering pet insurance to manage costs.

“Proactive care is vital regardless of location – annual wellness exams, vaccinations, and preventative treatments can catch problems early. With telehealth becoming more available, pet owners in underserved areas can now access veterinary guidance remotely, potentially avoiding unnecessary emergency visits and improving their pets’ quality of life.”
